    I, _________________________of legal age, single, Filipino and a resident of Barangay Villa Sta.
    maria, Zamboanga City, after having been duly sworn to an oath in accordance with the law, hereby
    depose and say:

    1. That I was unemployed during my stay at Tawi-Tawi, Municipality of Bongao;


    2. That I applied for a TAX IDENTIFICATION NUMBER (TIN) at BIR,Bongao Tawi-Tawi RDO
    096;
    3. That as such, I was issued a Tax Identification Number (TIN) 762 5252 49000 by the
    aforementioned office;
    4. That at present I am residing at 542 Gumamela Street Villa sta. maria, Zamboanga City;
    5. That for the above-mentioned reason, I hereby request the BIR to transfer my TIN from
    RDO 096 to RDO 93A as I am now residing at _____________________, which place is
    under the jurisdiction of RDO 93A;
    6. That I executed this letter freely and voluntarily in order to attest to the truth and
    veracity of the foregoing and for whatever legal intents and purposes this may serve.
