JOINT AFFIDAVIT
(RE: Change of Venue for Marriage Solemnization)
We, RALLPHY A. ALDANA and ABIGAIL S. PAYOYO, both of legal age, Filipinos, and
residents of #2364 Sta. Lucia, City of San Fernando, Pampanga, after having been sworn in accordance
with the law, do hereby depose and say:

1. That we are contracting parties who requested for marriage solemnization with Church of
Christ, City of San Fernando, Pampanga.

2. That we intend to be wed on December 19, 2022 in “THE HAUSLAND PAMPANGA, the
PAVILION”, Panipuan, Mexico, Pampanga with the blessings of our respective parents, Mr.
& Mrs. Rolly and Belinda Aldana and Mr. & Mrs. Cenon and Marlyn Urbano.

3. We further intend that our garden wedding ceremony be solemnized by Bro. Felipe M.
Palomar, Church Minister of the Church of Christ.

4. That we are executing this affidavit to attest the truth of the foregoing statements for the
purpose of compliance as regards to the change of venue for marriage solemnization as
required pursuant to Article 8 of the Family Code of the Philippines and for all legal intents
and purposes it may serve.
