5 USING THE SYSTEM

1. Go to following link:https://smartforms.mohap.gov.ae/GSC/AppPages/ApplicationWorklist
2. Click on start Service
3. System will prompt you to show Following log-in form:
4. Log-in using UAEPASS or credentials (if user has no credentials, he/she can
sign up)

6 APPLICATIONS FOR A GOOD PROFESSIONAL CONDUCT CERTIFICATE FOR
PROFESSIONALS IN THE GOVERNMENT HEALTH SECTOR – STEPS

By Using this Service, the applicant can create a new request for Submitting
Applications for a Good Professional Conduct Certificate for Professionals in the
Government Health Sector on MOHAP.

6.1 CREATE NEW REQUEST

After Login into Service, System prompt to the following landing screen:

Once the applicant clicks on the “Create New Application” button, the below
screen will be shown where the applicant needs to choose the “Employer type”
then click on “Proceed Application” button:

6.1.1 Fill details

Applicant then will be prompted to fill the required data. All fields with red start *
are mandatory where others are optional. Once complete filling the information,
the Applicant needs to click on “Save” button.

After reviewing the Application data, the Applicant need to fill the Experience
Details by clicking on “Add” button under “Experience Details” section where All
fields with red start * are mandatory while others are optional. Once completing
filling the Experience details, the Applicant should click on “Save” button.

6.1.2 Attachment

After viewing the application data and filling the Experience Details, Applicant
must upload documents by clicking on “Add” button under “Attachments”
section.

After clicking on “Add” button, Applicant must choose the Attachment type and
add attachments then click on “Save” button.

Once complete adding all the required documents, the Applicant must click on
“Submit” button. Applicant then need to complete the payment in order for the
application to be sent for business team reviewal and approval.
