 Once the problem is identified, there must be a clear

 The objective of cause analysis is to identify the root


cause(s) of the problem.
 Whether to eliminate or minimized
 There are many tools for cause analysis such as:
 Problem Tree
 Fishbone diagram (ISHIKAWA)
 5-Why
 Etc.

times to uncover the real root
cause(s) of a problem.
 Investigate the causes at each
level to find out which ones are
facts that lead to the root cause.

Example: 5-Whys

W1 Why material was not reached on time? A1 Because it was dispatched late.

W2 Why material was dispatched late? A2 Material was not ready on time.

W3 Why material was not ready on time? A3 Material specification reached late.

W4 Why specification was reached late? A4 Material specification was not sent with P.O.

W5 Why specification was not sent with P.O? A5 Purchase person missed to send it together?

discrepancy
 The cause can be
addressed directly
 The cause is an element
for which you can plan
countermeasures.
