EMPLOYEE BENEFITS AND COMPENSATION SURVEY

The results of this survey will enable you to assess the effectiveness of the employee benefits and
compensation program of your organization during your time as an organizational member at
work. This survey will also allow you to assess the strengths and weaknesses of the employee
benefits program. The respondents of this assessment will remain anonymous. The results of this
survey will only be used to inform organization about the effectiveness of their employee
benefits program.
