CHECKLIST ON BED BATH

Assessment

1. Assist the client’s tolerance for activity.

2. Assess room temperature and ventilation

Planning

3. Wash hands.

4. Gather all necessary equipment.

Implementation

5. Identify the client and explain the procedure.

6. Provide privacy.

7. Offer bed pan or urinal.

8. Adjust bed to a comfortable working height position.

9. Place bath blanket over the client before removing


the top sheet.

10. Remove client’s gown ensuring he/she is covered


with blank blanket.

11. Pull side rail up and fill basin with warm water.

12. Place client in a semi-Fowler’s position and


remove pillow (if permissable).

13. Make a mitt of the wash cloth.

14. Wash client’s eyes from the inner canthus towards


the outer canthus.

15. Wash client’s face, ears and neck. Rinse pat and
dry.

16. Wash am away from you, distal to proximal


including axilla, using long firm strokes. Rinse and dry.

17. Repeat the procedure on the arm near you.

18. Wash client’s hand in basin. Rinse and dry.

19. Wash chest and abdomen using long firm strokes.


Rinse and dry.
20. Expose one leg at a time and wash leg far from
you with long firm strokes. Rinse and pat dry.

21. Repeat procedure on the leg near you.

22. Immerse and wash client’s feet in the basin. Rinse


and pat dry.

23. Replace water..

24. Turn patient on his/her side and place towel


alongside of the back. Wash the back and buttocks
with long firm strokes. Rinse and pat dry.

25. Place the patient in supine position and cover with


bath blanket and then wash the perineal area. Rinse
and pat dry. If patient is able, place the towel under
the buttocks and bring basin and soap within client’s
reach. Instruct client to complete the bath him/herself.

26. Have the client wear new gown and comb hair.
Apply lotion if desire by client.

27. Remove equipment used and store them in a


proper place.

28. Wash hands.

Evaluation

29. Evaluate the client’s comfort.

Documentation

30. Document the procedure to include the type of


bath administered and client’s response.
