I, FRANNIE PANCHO TAN, Filipino, of legal age, and with business address at 280
Sitio Talisay, Bato, Toledo City, after having been sworn in accordance with law, hereby depose
and state that:

1. I am the Corporate Secretary of KOINONIA FUNERAL HOMES OPC (the

“Corporation”), a corporation duly organized and existing under and by virtue of the
Philippine laws with principal business address at Sitio Talisay, Koinonia Group of
Believers Ministries Building, Bato, Toledo City, Cebu, Region VII.

2. On 01 December 2024 there was a special meeting of the Board of Directors of the
Corporation where a quorum was present and existed throughout;

3. On the said meeting, the following resolution was adopted by the Board of Directors after
motion duly made and seconded:

“RESOLVED, as it is hereby resolved, that the Corporation shall register and apply for
registration with the City of Cebu and The Bureau of Internal Revenue;

RESOLVED, further that the Corporation shall authorize FEO UBAS to act as the duly
authorized representative of Koinonia Funeral Homes OPC and to thereby sign, execute,
and receive the necessary application and forms, transact with the said government
agencies, as well as all other documents incidental and necessary to effect the
registration.

4. Affiant further certifies that the foregoing resolution has not been revoked, superseded or
amended, and are now in full force and effect as of this date.
