REQUIREMENTS FOR DEATH SINGLE

1. Application for Provident Benefits (APB Claims form)- (1 ORIGINAL COPY)
2. LCRO/PSA authenticated Death Certificate of Member- (1 PHOTOCOPY, 1 original for
MRI)
3. LCRO/PSA authenticated Birth Certificate of Member- (1 PHOTOCOPY)
4. LCRO/PSA authenticated Marriage Contract of member’s parents- (1 PHOTOCOPY)
5. CENOMAR of deceased Member from PSA- (1 PHOTOCOPY)
6. Proofs of Surviving Legal Heirs- (1 ORIGINAL COPY)
7. If with Illegitimate children:
a. Notarized Affidavit of Cohabitation attested by Two Disinterested Person (two
brgy. Officials)
b. PSA authenticated Birth Certificate of children.
c. Declaration of Guardianship if with minor children.
d. PSA authenticated Marriage Contract of Member’s daughter if married.
8. If parents were deceased:
a. LCRO/PSA authenticated Death Certificate of Member’s mother/father
(1 PHOTOCOPY)
b. LCRO/PSA authenticated Birth Certificate of Member’s brothers/sisters
c. LCRO/PSA authenticated marriage Contract of Member’s sisters if married
d. LCRO/PSA authenticated Death Certificate of member’s brothers/sisters if with
deceased brothers/sisters.
9. Joint affidavit of Abandonment attested by two disinterested person (brgy officials)
that the children abandoned by parent w/ 2 valid ID cards of witness.- (1 ORIGINAL
COPY)
10. Joint affidavit from two (2) disinterested Person (two Brgy. Officials) attesting that
he/she died single and w/o children- (1 ORIGINAL COPY)
11. If with Discrepancies: submit Affidavit of Discrepancy from two (2) Disinterested
Person- (1 ORIGINAL COPY)
12. Two valid I.D. cards of member and claimant/parents – (2 PHOTOCOPY)
13. SPA/NOTARIZED Waiver of Rights w/ their two valid ID cards (optional)- (1
ORIGINAL COPY)
