PRE-EMPLOYMENT REQUIREMENTS CHECKLIST

On your first day, please report at least 1 hour before your shift and be in business casual attire. If you're unable to
make it on your onboarding schedule without notification, we'll regretfully rescind your job offer.

TO BE SUBMITTED UPON CONTRACT SIGNING

1. Background Investigation Form

2. ID Form with 2pcs. 2x2 photos with white background

3. Signed Employment Contract

4. Signed Data Privacy Consent Form (Employee & Dependets)

5. Onboarding Form

6. NBI Clearance/Receipt

7. Security Bank (for Payroll)

NON-NEGOTIABLE REQUIREMENTS
(To be submitted 1-5 days from Job Offer Date) Deadline____________

1. SSS ID
- 2 copies of any of the following:
SSS ID/E1/E4/E6/Static Information Printout from SSS Website https://www.sss.gov.ph/
visit SSS office for print-out assistance of your static information ; loan voucher (for verification)
NOTE: UMID is not acceptable
If you have an existing SSS loan, submit your most updated Statement of Account (can be accessed in SSS website).
2. Philhealth (PMRF)
IF with PH number
- 2 photocopies of MDR/PhilHealth ID
IF without PH number
- Completely filled out M2 Form via online
http://www.philhealth.gov.ph/ ; print out the PMRF with the PH number reflected

*Please note that PMRF forms will be submitted to us for processing. PMRF form is included in the Job Offer Kit

3. Philhealth Certificate of Contributions


- visit the PhilHealth website www.philhealth.gov.ph/
- Proceed to Online Services -- Member Inquiry

- Activate your account using the link sent by Philhealth to your personal email address
- Click on Premium Contributions and print

4. BIR/TIN ID
IF WITH TIN
- 2 photocopies of any of the following:
previous BIR stamped 1905, 1902, 2305 or TIN ID (2316 is not acceptable)
- Completely filled out 2305 form (for update of employer-included in the Job Offer Kit)
- Completely filled out & stamped 1905 form for RDO Change/transfer ; if unsure check with previous
HR/employer for RDO code/branch
IF WITHOUT TIN
- Completely filled out 1902 form (for new application & attach a copy of NSO Birth Certificate)

*Please note that BIR forms will be submitted to us for processing.

5. PAGIBIG
- 2 Copies of Pag-ibig ID/Loyalty ID or a copy of MID number/
loan voucher, Statement of Account, Updated Payments Made (for verification purposes)

6. 4 copies NSO Birth Certificate and Marriage Certificate if applicable

7. Medical Exam Completion Slip

8. Health Insurance Form

9. 4 copies NSO Birth Certificate of dependents

10. Cedula
