AFFIDAVIT OF CANCELLATION
WITH NO FINANCIAL OBLIGATION

I, TRISHA MARIZ SOBIACO, of legal age, single, Filipino Citizen, and a resident of
BLDG. 19 UNIT 204 AMVA HOUSING LAMESA STREET BARANGAY UGONG
VALENZUELA CITY PHILIPPINES, after having been sworn in accordance with the law do
hereby depose and state, THAT;

1. I am the same and identical person who executed this Affidavit;

2. I am a registered owner of a business under the Trade Name GLOW GLAM

MNL HEALTH AND BEAUTY PRODUCTS TRADING located at UGONG
CITY OF VALENZUELA, NCR THIRD DISTRICT- NCR- NATIONAL
CAPITAL REGION;

3. The above-stated business was duly registered at the Department of Trade

and Industry (DTI) on March 13, 2021 under Business Name No. 2755464
Valid until March 13, 2026;

4. At present, I am causing the cancellation of the said registered business due

to no plans to continue the business;

5. As far as my personal knowledge as the registered owner, I attest to the fact

that GLOW GLAM MNL HEALTH AND BEAUTY PRODUCTS TRADING has
no outstanding financial obligation to any individual public or private
corporation or entities;

6. I am executing this affidavit to the attest the truthfulness of the foregoing

declaration under penalties of perjury to respectfully request the DTI to
CANCEL Certificate Registration No. 2755464 and for all legal purposes.

7. As such, I am executing this Affidavit of Loss to attest to the truth of the

foregoing for the issuance of a new copy of receipt, in lieu of the one that
was lost.
