CARATAGAN PIO DURAN ALBAY ON DECEMBER 14, 2022.

Payment of Yearly Dues

Call to Order:The meeting was called to order at 1:00 o’clock in the afternoon.

Roll Call: The roll call was made and we have the attendance indicated.

Reference of Business:

The meeting was conveyed at 1:00 in the afternoon. Upon quorum was
constituted, President Nancy Burac presented the agenda for today’s meeting.
The Presiding Officer state that it is very important that all Barangay Health
Worker, accredited and non- accredited must be registered in amount of Two Hundred
Seventy (P270.00) pesos only and it also serve as a payment for the yearly dues. She
also added that the member will benefits from this because it’s member will receive an
incentive an amount of One Thousand (P1,000.00) pesos during end of the year and
etc., And it was mass collectively seconded by it and Unanimously approved by the
body.

Adjournment: After due consideration and deliberation the Presiding Officer

Nancy Burac found no objection on the motion being relevant to the given agenda
And since there being no other matter to be discussed the meeting was an
adjourned at 3:00pm.
