AFFIDAVIT OF DISCREPANCY

I, EDUARDO DEGAMON SAMSON, Filipino, single, and a resident of P-

Silangan, Barangay Agusan Pequeño, Butuan City, after having been duly sworn to
in accordance with law, do hereby depose and state THAT:

1. There is a DISCREPANCY from the Department of Social Welfare and

Development on their Social Amelioration Program List, in which my
FIRST NAME was erroneously entered therein as “EDGARDO”, when in
truth and in fact it’s “EDUARDO”, as evidenced by my Professional
Driver’s License and Barangay Certification;
2. For all intents and purposes, the name “EDGARDO” and “EDUARDO”
refer to the name of one and the same person, EDUARDO DEGAMON
SAMSON;
3. I am executing this affidavit to attest to the veracity of the foregoing for
whatever legal purpose it may serve.
