Introduction
● You are being asked to be in a research study about the variables affecting Customer
Satisfaction in coffee shops: Quality of Product, Quality of Service, and Ambiance.
● You were selected as a possible participant because you are qualified
● We ask that you read this form and ask any questions that you may have before agreeing to
be in the study.
Purpose of Study
● The purpose of the study is to determine which of the independent variables (Quality of
Product, Quality of Service, and Ambiance) have the most impact on Customer Satisfaction.
● Ultimately, this research may be presented as a paper
Confidentiality
1. The records of this study will be kept strictly confidential. Research records will be kept in a
locked file, and all electronic information will be coded and secured using a password
protected file. Audio and video of the interview will be recorded for transcription purposes,
accessed only by the researchers, and will be securely disposed of once it serves its purpose.
We will not include any information in any report we may publish that would make it
possible to identify you.
Payments
● The subject of this interview will not receive any financial compensation.
