De La Salle-College of Saint Benilde (DLS-CSB) respects your right to privacy and is


committed to protect the confidentiality of your personal information. DLS-CSB is bound to
comply with the Data Privacy Act of 2012 (RA 10173), its implementing Rules and Regulations
and relevant issuances of the National Privacy Commission.

It has adapted reasonable administrative, physical and technical measures to prevent loss, misuse
and alteration of the information under our control. However, no method of transmission over the

Introduction
● You are being asked to be in a research study about the variables affecting Customer
Satisfaction in coffee shops: Quality of Product, Quality of Service, and Ambiance.

● The purpose of the study is to determine which of the independent variables (Quality of
Product, Quality of Service, and Ambiance) have the most impact on Customer Satisfaction.

● If you agree to be in this study, you will be asked to do the following things: answer the
survey questionnaire provided by the researchers. You are allowed to express your own
opinion in every situation and questions that will be asked in the questionnaire.

Confidentiality

1. The records of this study will be kept strictly confidential. Research records will be kept in a
locked file, and all electronic information will be coded and secured using a password
protected file. Audio and video of the interview will be recorded for transcription purposes,
accessed only by the researchers, and will be securely disposed of once it serves its purpose.
We will not include any information in any report we may publish that would make it
possible to identify you.

Payments
● The subject of this interview will not receive any financial compensation.

Right to Refuse or Withdraw


● The decision to participate in this study is entirely up to you. You may refuse to take part
in the study at any time without affecting your relationship with the investigators of this
study or DLS-CSB. Your decision will not result in any loss or benefits to which you are
otherwise entitled. You have the right not to answer any single question, as well as to
withdraw completely from the interview/ survey at any point during the process;
additionally, you have the right to request that the interviewer not use any of your interview
material.

Right to Ask Questions and Report Concerns


● You have the right to ask questions about this research study and to have those questions
answered by me before, during or after the research. If you have any further questions about
