 Filipino Citizen, resident for at least 5 years
 Licensed Pharmacist, preferably masteral degree or its equivalent

 Not convicted of a crime
 Member of APO (Accredited Professional Organization) such PPHA (Philippine Pharmacists
Association)at least five years
 Not a member of the faculty of any school

 An applicant who failed in the licensure examination third time shall not allow to
take the next succeeding examinations without a refresher program

Penal Provisions

 To pay fine of not less than 250,000 but not exceeding 500,00, or
imprisonment of not less than 1 year and 1 day but not more than six years
 Displaying COR where the pharmacist is not employed
 Dispensing in a place not licensed by FDA
 Compounding and dispensing not in accordance of cGMP (current Good
manufacturing practice)
 Selling to unlicensed pharmaceutical
 Adultering, misbranding
 Practicing pharmacy with an expired, suspended or revoked license
Penal Provisions

 To pay fine of not less than 100,000 but not exceeding 200,00, or
imprisonment of not less than 30days but not more than 1 year
