AFFIDAVIT OF EXPLANATION

I, TERESITA B. SALAZAR, of legal age, American citizen, and with residential

address at 2942 North 40th Lane, Phoenix, Arizona, United States of America, after having been
duly sworn to in accordance with law, depose and state that:

1. I was granted a Certificate of Naturalization (“Certificate”) by the United States of

America last November 8, 2019, with No. 41448988, and attached hereto as Annex “A;”
2. When I travelled to the Philippines last December 2020, I failed to bring the original
copy of the abovementioned Certificate but was able to bring a photocopy thereof;
3. The original copy of the Certificate is at my abovementioned residential address; and
4. As such, I am executing this Affidavit of Explanation to attest to the truth of the
foregoing, and to support my application for dual citizenship at the Bureau of
Immigration.
