Mr. Bhubneshwar Kumar arrived promptly with his son to undertake the assessment. He was casually dressed. He

maintained eye contact and answered questions clearly. His mood and affect were interactive. During the assessment,

rapport was established easily. He maintained motivation throughout the assessment.

No family history is reported. He has a physical history of paralysis. The communication is clear. His sleep pattern is

not healthy. Constipation is present as a complaint of bowel and bladder. The consistency of the diet is regular.

His score in the assessment indicates mild cognitive impairment. He has high psychological distress and low well-

being. The concern area on which we need to work is – Executive memory and visuospatial. The age-associated

memory impairment is present. As per assessment mild dementia also exists with stage 2.

Suggestive measures can be: -

- Ongoing medication and consultancy from Dr. Parveen Gupta, Neurologist, Fortis Hospital.

- Psychotherapeutic Interventions -

1) Cognitive Behaviour Therapy - helps people learn how to identify and change the destructive or

disturbing thought patterns that have a negative influence on their behavior and emotions. CBT can

teach individuals strategies to cope with and manage cognitive symptoms. CBT can help individuals

identify and challenge negative thought patterns and develop healthier ways of thinking and

responding to their cognitive challenges.

2) Cognitive stimulation therapy - CST provides structured activities designed to stimulate various

cognitive functions, including memory, attention, language, and problem-solving. These activities

can help individuals with MCI exercise their cognitive abilities and maintain cognitive function to

the best of their ability.

I would conclude, given the evidence, that it is likely Mr. Bhubneshwar will require reasonable interactional

adjustments within the due process outlined above, and an intermediary treatment would be necessary to assist

in this regard.
