AFFIDAVIT OF VOLUNTARY SURRENDER

I, TINKER BELL, of legal age, Filipino and a resident of the City of

Dreams, Philippines after having been sworn to in accordance with law
hereby depose and state that:

1. I am the Accused in Criminal Case No. Infinity, which is

pending before the Regional Trial Court, Branch 0, City of
Dreams;

2. Said case was promulgated via trial in absentia on September

28, 2024;

3. This is also to manifest that I was not able to attend said

hearing because I was sick. Attached herein as Annex “A” is
my medical certificate from the Beauty Clinic of Doctor Lopez;

4. As such, I hereby manifest that I am voluntarily surrendering

myself and I am voluntarily surrendering myself before the
Honorable Court through the Philippine National Police, for the
filing of my motion for leave of court to be allowed to avail of my
post sentence remedies. Attached herein as Annex “B” is my
certificate of detention.
