On January 11th, 2024 time 09:15 AM while BVI-
HSE Performed daily site inspection, found unsafe
action PIBK workers who were doing touch-up
work did not wear special PPE (full-body harness)
when working on scaffolding and also used
scaffolding that was not proper and was not
checked/verified/tagged (green) by the certified
scaffolding inspector

On January 11th, 2024 time 09:16 AM while BVI-
HSE Performed daily site inspection, found unsafe
condition there was no protection installed in the
dangerous hole area and there was also no safety
sign posted as a warning to other workers

On January 11th, 2024 time 13:38 PM while BVI-
HSE Performed daily site inspection, found unsafe
condition tie rod steel used for formwork was not
immediately taken down, potentially falling down
and hit the workers below
