GRADE 10 ACCOMPLISHMENT REPORT
S/Y 2021-2022
I. Introduction
This year-end report provides an overview of the accomplishments of Grade 10 Level in Gonzalo Aler National High School as
well as the activities initiated through the collaborative efforts of School Head, teachers, students, Parents-Teachers association,
and the community leaders.
II. Enrollment
A. We conducted house to house enrollment and orientation of parents before the opening of classes.

III. Failure and dropped out
No failure and no dropped out from grade 10 learners.
IV. Passing Rate
The 249 enrolled learners in grade 10 have completed the basic requirements prescribed by the Department of Education under K-
12 curriculum or 100 percent passing rate.

V. Issues/Challenges Encountered

1. Students are not able to take the modules at school because neither a parent nor a guardian is available and have a family
problem.
2. Unwillingness of the students to complete the modules because they prefer face-to-face instruction.
3. Unnamed output has been received.
4. There are parents who are answering their child's module.
5. There are instances that the modules they received is doubled or they received the same modules as if the teachers are not
checking the records that they released.
6. Learners unable to submit the outputs on scheduled retrieval of modules due to financial matter.
7. Learners unable to answer some of activity from SLM due to difficult instruction and presentation of lessons.
8. Teacher’s difficulty in sorting learners’ outputs.
9. Poor internet connection which leads to the delay of the online class.
10. Learners are mostly expected to work with parents for daily necessities.
11. Lacking guidance in answering SLM due to educational status of parents/guardian.
12. Early Pregnancy.
13. Unable to attend on scheduled online class.
14. Not answering parallel assessment and not doing the performance tasks.
15. Learner don’t have the capacity to perform the performance tasks.
16. Misplace learners’ output.
17. Because of an unstable internet connection and the fact that some of the learners do not have a smart phone, some of the
Learners are unaware of the information delivered to group chat.
18. Some students are attending/joining late in online class via google meet.
19. Absenteeism of some learners due to early employment.
Third and Fourth Quarter
1. Some students are unable to participate in limited face-to-face activities due to work obligations.
2. Some students had forgotten about the classroom health protocol.
3. Many students in modular irregularly get modules due to different issues like sickness, early pregnancy and the like.

VI. Solutions Made
First and Second Quarter

1. Home visitations were conducted, and they were given written activities to accomplish from a variety of subjects.
2. Through messaging, encouraged them to complete the modules and emphasized the importance of completing this level.
3. Talked to his parents and made an agreement that they need to get modules and complete them on a weekly basis.
4. Posted the unnamed output to all sections' group chat and asked the student to message us if they owned it.
5. Talked to the student about the issue.
6. Explained that this happens from time to time because some of the activities in the modules are left unanswered, which is why
some teachers repeat the same modules.
7. The teachers allow learners for online submission of output via messenger.
8. The teachers made modifications on SLM and WHLP.
9. The teachers allow learners to comply on their incomplete requirements.
10. Teachers provide boxes per learning area for every section.
11. Share to the learners the recorded copy of discussion.
12. The teachers don’t give due date on submission of outputs.
13. The teachers provides soft copy of modules to learners.
14. Extended help to struggling learners.
15.Gave easy instruction to the learners during modular activity at home.
16. Continuous reminders on GC.
17. Encourage the learner to be grouped with his/her neighboring classmates in performing the tasks.
18. Monitoring tools were used to ensure that the modules given were correct.
19. Looked for strong signal and have enough load to sustain online class.
20. Always remind learners to join early or on time in online class.
Third and Fourth Quarter
1. Inform the students that he/she will participate in the modular distance learning program.
2. Constant reminder about the health protocol.

3. Constant communication with the learners and giving them soft copy of modules.

VII. Mode of Release and retrieval of Modules


The parents/guardian will sign first to the logbook and go through minimum health standard check like Temperature check and
Hand sanitation.
a. Module Pantry
Here you will find the modules, Learning Activity Sheets, and parallel assessment of the different learning areas/subjects. We
made this to easily sort and find the needed materials.
b. Answer sheets and Retrieved Modules Pantry
We have separate boxes for the answer sheets and the retrieved modules, and they are supposed to place their output and modules
in the appropriate compartments. After that, the teacher who has been assigned to the class will hand out the necessary modules,
LAS, and parallel assessment for the week.
c. Teacher’s Monitoring Tool for Learners Performance
This is a tool used in monitoring the distribution of SLMs/LAS and retrieval of learner’s outputs and informing parents and
learners on the rating/ scores in module activities in all learning areas of the modular distance learning. This tool covers a kind of
intervention to be given to the learners who needs improvement in the module activities. Progress report also reflected in the tool.
It is also a reference of the school in classifying the learners whether a SARDO or not since it contains their profile.
VIII. Student Development
a. Parallel assessments were administered to students enrolled in modular distance learning, as well as diagnostic and
summative assessments to students who attended limited face-to-face classes.
b. Students with behavioral issues were provided counseling.
c. Motivated students to study.
d. Conducted home visits to students who were unable to return the modules due to the absence of a parent or guardian or a
family problem.

h. Encouraged and motivated students to take part in extracurricular activities and programs offered by the school.
i. Organized the classroom officers.
j. Conducted Oplan 6 in 1 school activity which the students undergo in the kumustahan, numeracy, literacy, PhilIri, BMI test
and Fayaman distribution of modules and orientation in every Barangay.
k. Conducted orientation for limited face-to-face classes.
l. Conducted limited face-to-face classes every first and third week of the month from third to fourth quarter.
m. Held a quarterly awards ceremony/Recognition Day to celebrate accomplishments.
n. Facilitated weekly remedial classes.
o. Learners in grade 10 were given career guidance as part of an effort to get them ready for the selection process for senior
high school tracks and strands.
p. Conducted Numeracy assessment.
q. 7th completion rites for grade 10 were held on June 27,2022, honoring the students' hard work and dedication to learning.
