Subject: Complaint Letter of Pending Request Document (Seaman's Book)

Dear Madam,

I am GILICE M. ERALDO and I am writing to make a formal complaint regarding my pending request of
document (Original Copy of my Seaman's Book) in your good office.

I have sent all the required original documents in your office last November 2020 through LBC for the
compliance of requirements on my next onboard cruise joining, but due to the COVID 19 Pandemic, onboard
jobs have been suspended and postponed. With such, my other documents has the need to be updated this
time and I need my ORIGINAL COPY OF SEAMAN'S BOOK to process my SIRB.

I have been trying to contact you since June 07, 2021 through calls, text and email but I have not recieved any
response from you regarding the said matter. I had already requested the said document in the company's
online site last June 21, 2021 but it was still pending until now.

Please do also consider that I am residing in a Province and have been there in Manila for almost a month to
reprocess all my joining requirements in your company, thus making it also difficult on my part to wait for too
long on that said one document. I am humbly asking your kindness and consideration to notice this matter
and give solution to my request as soon as possible to be able to know if I still need to stay here in Manila for
a long time or go home to my province.

I understand that you are required to respond formally to my complaint. I shall follow up this letter if I do not
hear back from you as soon as possible. In the meantime, if you need any further information from me,
