High-Potential property damage. Equipment and Vehicle collision
Description:
A wheel loader operator was engaged in routine road levelling activity in excavation area. A
mid-sized SUV driven by an engineer was passing through the work area and stopped at the
back of the wheel loader. At that time the wheel loader reversed and hit the vehicle resulting
damage to windshield, bonnet, and bumper.
Recommendations/Preventive Actions:
• As part of the Traffic Management Plan(TMP) ensure
that smaller vehicles are not required to go in to
areas where heavy equipment operate.
• If there is a need for smaller vehicles to go to areas
where heavy equipment normally operate then this
should be included in the TMP or a specific risk
assessment. Suggested control measure to assist
with risk mitigation are as follows;
• Full segregation, if segregation is not possible
then temporary suspension of operations is
required.
• Flags & flashing beacons to be utilized on
smaller vehicles and Pickups to aid visibility PHOTO
while working in the area
• Ensure suitable and sufficient barriers &
signage is placed.
• Regular defensive driving training to all
drivers and operators at site.
• If it is important to stop, stop vehicle in an
area very visible for the operator. Internal
