AFFIDAVIT OF DENIAL

I, CHRISTIAN LEAÑO DIZON, of legal age, single, a Filipino citizen and presently
residing at #247 Sitio 3, Pulong Gubat, Candaba, Pampanga, after having been duly sworn
to in accordance with law, do hereby depose and state that:

1. My complete and true name is CHRISTIAN LEAÑO DIZON, as evidenced by my

Unified Multi-Purpose ID issued by the Social Security System, Philippines;

2. In my application for NBI Clearance, I was informed that one “CHRISTIAN

DIZON”, has pending criminal case filed in MTCC Mabalacat City, Pampanga with
criminal case no. 17-008;

3. I hereby attest that I am not this “CHRISTIAN DIZON” who is being prosecuted
for the abovementioned case, and further declare that I have never been charged
of any criminal case anywhere in the Philippines;

4. I understand the contents of this affidavit and I know that I can be liable for
perjury if proven later that I am not telling the truth;

5. I am executing this affidavit to attest to the truth of the foregoing facts and for
whatever legal purpose this may serve.
