MEDICAL CERTIFICATE

This is to certify that Gurpreet Singh (DOB:-08 th Jan. 1986) S/o Kewal Singh R/o Village Ramidi, Post
Office Daduki Patti, Tehsil and District Kapurthala, Punjab, India has visited my hospital on 11 th
September 2022 on the reference of J.J. Trauma Centre, Thind Hospital. After thoroughly examined
and x-ray of his mouth, it was found that his jaw almost injured and swelled. Even, I tried to save his
teethes but I could not save his two front teethes as I fixed wire over his all teethes and implant two
new teethes after pulled over his two front teethes which was totally cracked. Even, I said him to come
again in the hospital after one week for proper treatment of his teethes. He again came my hospital on
19th September 2022 and total three months had been taken for his proper treatment. The following
medicines has been given during his treatment.

1. Ibuprofen Tab. 400 mg

2. Anesthetics Injection
3. Amoxicillin Cap. 500 mg
4. Benzamycin Gel 20 gm
