    LETTER OF REQUEST

    Hi Converge,

    I am requesting the termination of my Internet Connection Plan 2899 immediately. The


    reason for termination is because I waited for almost a month for my internet to be relocated to
    my new office address and I cannot wait any longer. I have given you all the required documents
    necessary for the transfer. No converge staff is updating me or telling me what’s missing in the
    requirements. I believe I have explained all and answered all the questions asked of me. I do not
    want to pay another month of subscription for the service which you are not giving me.
