SWORN DECLARATION WITH UNDERTAKING
I, VICENTE E. ROA, JR., married, of legal age, authorized representative of
EPICUREAN PARTNERS EXCHANGE INC., and COFFEE MASTERS, INC. with business
address presently at 10th Floor, BA Lepanto Bldg., 8747 Paseo De Roxas Ave., Salcedo Village,
Makati City, after having been sworn to in accordance with law, hereby depose and say that:
1. That I am the authorized representative of KENNY ROGERS ROASTERS located at 2016
2/F Ayala Malls, Manila Bay, Macapagal Blvd., Tambo, Parañaque City (branch); KENNY
ROGERS ROASTERS located at G/F Unit A #317 Aguirre Ave., BF Homes, Parañaque
City (branch); and SEATTLES BEST COFFEE located at G130 G/F Ayala Malls, Manila
Bay, Macapagal Ave., Tambo, Parañaque City (branch)

2. As we requested for the Statement of Account (SOA) 3 rd quarter and your office required
some documents we are requesting your good office to give us the SOA for the 3 rd quarter
since the due date is until July 20, 2023 only. With this, I hereby undertake that we will
provide the requested document from the Bureau of Internal Revenue (BIR) RDO 125LTAD
located in Quezon City and RDO 53B Muntinlupa on or before the 4th quarter. As the
Certified True Copies take some time to process;

3. I am executing this UNDERTAKING to attest to the truth of the foregoing and for whatever
lawful purposes this may serve.
