AFFIDAVIT FOR CHANGE OF NAME

By this deed I, the undersigned MR. MOHAN SHYMBIHARI

KANOJIA, occupation: ____, resident of 48/2 Snehal CHS
Tarun, behind Suresha hotel, Bharat Society Chakala, Sahar,
Sahar P & T Colony, Mumbai-400099, Maharashtra, do hereby
solemnly declare, affirm and state as under:
1. That in the Agreement for Sale made at Bombay at 1 st day
of August One thousand nine hundred and seventy eight my
name is wrongly mentioned as MR. MOHAN
SHYAMBIHARI KANOJIYA and the same needs to be
rectified and corrected as MR. MOHAN SHYMBIHARI
KANOJIA as mentioned in my Aadhar Card bearing no.
5689 1197 3817
2. That, I am making this Affidavit, to state, declare and to
correct my wrongly mentioned name from MR. MOHAN
SHYAMBIHARI KANOIJIYA as taken in the Agreement
for Sale to be corrected and read as MR. MOHAN
SHYMBIHARI KANOJIA as stated in my Aadhar card.
3. That for and on behalf of myself, I wholly renounce,
relinquish and abandon the use of my wrongly mentioned
name MR. MOHAN SHAYMBIHARI KANOJIYA and
in place thereof I do hereby assume from this date the name
MR. MOHAN SHYMBIHARI KANOJIA so that I may
hereafter be called, known and distinguished not by my
former name, but assumed name MR. MOHAN
SHYMBIHARI KANOJIA.
4. That for the purpose of evidencing such, my determination
declare that I shall at all times hereafter in all records, deeds
and writings and in all proceedings, dealings and
transactions, private as well as upon all occasions whatever
use and sign the name of MR. MOHAN SHYMBIHARI
KANOJIA as my name in place and in substitution of my
 former name MR. MOHAN SHYAMBIHARI
KANOJIYA.
5. That I expressly authorize and request all persons in general
and relatives and friends in particular, at all times hereafter
to designate and address me by such assumed name of MR.
MOHAN SHYMBIHARI KANOJIA accordingly and all
the records where it is wrongly mentioned be corrected as
MR. MOHAN SHYMBIHARI KANOJIA.
6. I state that “MR. MOHAN SHYAMBIHARI
KANOJIYA” (earlier name) and “MR. MOHAN
SHYMBIHARI KANOJIA” (present name) is the name
of one and the same person and that is myself.
7. I, hereby undertake to provide any other Identity proof of
myself if required by the concerned authority for the
purpose of verifying my name.
8. I further state, declare and confirm that, whatever is stated
in the foregoing paragraphs of this affidavit is true and
correct and to the best of my knowledge and belief.
