AFFIDAVIT

I, the undersigned MOHANAN SHANKARAN, age:65 years, an adult,

Indian Inhabitant, residing at Building No. 36, Room No. 2535, M.H.B
Colony, Abhyudaya Nagar, Veer Shrikant Keshav Hadkar Marg, Near
ParelTank Road Post Office, Kalachowki, Mumbai-400033, do hereby
solemnly declare, affirm and state as under:
1. That my daughter MS. SUCHITRA MOHAN PANICKER has
applied for Fresh Passport under Passport Application bearing no.
BO30665795 in which my name, due to inadvertence, is wrongly
mentioned as MOHAN PANICKER and the same needs to be
rectified and corrected as MOHANAN SHANKARAN as
mentioned in my Aadhar Card bearing no. 758860884543 (Hereto
annexed and marked as “Exhibit-A” is a copy of my Aadhar
card).

2. That, I am making this Affidavit, to be submitted by my daughter

Ms. Suchitra Mohan Panicker to the Passport Authority/Concerned
Authority to correct my name from MOHAN PANICKER to be
corrected and read as MOHANAN SHANKARAN.

3. That for the purpose of evidencing the same, I declare and confirm
that my daughter shall at all times hereafter in all records, deeds
and writings and in all proceedings, dealings and transactions,
private as well as upon all occasions whatever use my name as
MOHANAN SHANKARAN in place and instead of MOHAN
PANICKER.

4. I state that MOHAN PANICKER (wrongly mentioned name) and

MOHANAN SHANKARAN (present name) is the name of one
and the same person and that is myself.

5. In the circumstances as aforesaid, my name be corrected, rectified

and adopted as MOHANAN SHANKARAN in the said Passport
Application.

6. I, hereby undertake to provide any other Identity proof of myself if

required by the concerned authority for the purpose of verifying
my name.
 7. I further state, declare and confirm that, whatever is stated in the
foregoing paragraphs of this affidavit is true and correct and to the
best of my knowledge and belief.
