NAME OF EMPLOYEE BEING EVALUATED N. CARREON PERIOD COVERED JAN. – JUN.

‘22

What was easy for you to accomplish? Why?

The accomplishment that was easy for me are:

1) Monitoring of COI template, issuance of internal CAR, follow-up of issued Internal CAR.
2) Validate the complaint/rejection report of the client.
3) Investigating the root cause of the complaint/rejection and gives a recommendation on how to prevent the
issue with the collaboration of responsible process owner/department.
4) Consolidate a DMS report and communicate with the top management on weekly report.
5) Conduct a cGMP audit in the plant to have a good working environment.
6) Monitored of controlled documentation information and maintenance of QMDS.
7) Observed a Client’s audit on how they performed and to learn more on a process of auditing.

What was difficult? Where are you having problems succeeding? Why?

1) Requesting of client’s specification for a new item that has been pending with TSG/Sales. Following-up the
TSG/Sales of the availability of the specification.
2) Overdue internal CAR because some of the department is busy and they can’t return the issued CAR on/before
the due date and as part of my job, I will give them an extension to answer the CAR.
3) Some of the complaint/rejection report from the client has not attached the actual sample of the defects found
so we are hard to validate if their claim of report is necessary. With the help of sales to coordinate to our clients
for the actual sample of the reported issue we can easily validated their report and do a necessary investigation
and corrective action and preventive action.

How can I (the Superior) help you better?

I am getting their full support in every project I made and guide me on how to report confidently on a certain problems.
Giving advises on how to do my job easily.
I will always be grateful in every decision I made because my superior is very supportive not only my work but also in my
career and personal growth.
