-Deficient Fluid Volume related to active fluid loss as evidenced by diarrhea

-High
-Fluid volume deficit is a condition where the fluid output exceeds the fluid intake. It occurs
when water and electrolytes are lost as they exist in normal body fluids
-The patient will be able to have normal bowel movements and be free from dehydration.

S: The patient verbalized “ Gikalibanga ko tapos basa permi akong tae”

O: > Dry skin


> Dry lips
> Fever
> Appeared weak

Short: After 4 hours of nursing intervention the patient will be able to understand the cause of
diarrhea and have adequate fluid balance.

Long: After 3 days of nursing intervention the patient will have normal bowel movements, be
free from fluid volume deficit, and maintain normal fluid intake.

Independent: Monitor vital signs


- Provides baseline data

Monitor of fluid intake and output


- To detect early signs of dehydration

Encourage to increase fluid intake


- For hydration

Establish rapport
- To facilitate cooperation and gain patient's trust

Provide health teachings


- To promote optimum wellness

Dependent: Antibiotics given as ordered


- To aid in preventing infection

Vitamins are given as ordered


- To aid in the general health of the patient

S: Goal met, after 4 hours of nursing intervention the patient understands the cause of diarrhea
and has maintained fluid intake.

L: Goal met, after 3 days of nursing intervention the patient no longer has diarrhea and is free
from dehydration.
