1 .म Somasree Maity एत ारा घोषणा करता हूं िक म Humara India Credit Cooperative Society
Limited,Sahara Credit Cooperative Society Limited का एक ामा णक जमाकता/सदस्य हूं और मने इस आवेदन प म

ऊपर िदए गए िववरण के अनुसार इस सिम त म वैध जमा िकया है । मने उपयु सिम त क सभी योजनाओं म मेरे ारा जमा क गई सभी

जमारा शय का िववरण िदया है । उपयुक्त द सूचना मेरी जानकारी और िवश्वास म सत्य और सही है और मने अपने जमा और दावे का

सही सा ्य िदया है । उक्त जमारा श/जमारा शयां मेरे ारा Humara India Credit Cooperative Society Limited,Sahara

Credit Cooperative Society Limited म मेरे वैध आय से सहारा े डट कोऑपरेिटव सोसाइटी ल., लखनऊ; सहारायन

यू िनवसल मल्टीपपज़ सोसाइटी ल., भोपाल और हमारा इं डया े डट कोऑपरेिटव सोसाइटी ल., कोलकाता क दशा म िदनांक

22.03.2022 से पहले और स्टास मल्टीपपज़ कोऑपरेिटव सोसाइटी ल., हैदराबाद क दशा म िदनांक 29.03.2023 के पहले और
संबं धत आयकर कानून के अनुपालन म िकया गया है ।

1. Somasree Maity hereby declare that I am a genuine depositor/member of Humara India Credit

Cooperative Society Limited,Sahara Credit Cooperative Society Limited and have made legitimate

deposits in this Society as per the details given above in this application form. I have given details

of all the deposits made by me in the above Societies in all the Schemes. The aforesaid

information given above is true and correct to the best of my knowledge and belief and I have

provided true proof of my deposit and claim. The said deposit(s) were made by me in the Humara

India Credit Cooperative Society Limited,Sahara Credit Cooperative Society Limited out of my

legitimate income before 22.03.2022 in case of Sahara Credit Cooperative Society Ltd., Lucknow,

Saharayn Universal Multipurpose Society Ltd., Bhopal and Humara India Credit Cooperative

Society Ltd., Kolkata and before 29.03.2023 in case of Stars Multipurpose Cooperative Society

Ltd., Hyderabad and in compliance with relevant Income Tax Laws.

2. यिद मेरे ारा उपयु आवेदन प म दी गई कोई भी सूचना बाद म गलत या िमथ्या सािबत होती है, तो म लागू ब्याज रा श सिहत रीफंड

क रा श ‘सहारा-सीआरसीएस रीफंड खाता’ म वापस करने और ‘सहारा-सीआरसीएस रीफंड खाता’ म उक्त रा श क तपू त करने क

वघोषणा करता हूं । म ‘सहारा-सीआरसीएस रीफंड खाता’ ारा िनपटायी गई दावा-रा श क सीमा तक के दावे को िकसी अन्य मंच म नह

उठाउं गा/उठाउं गी ।

2. If any of the information given by me in the above application form is subsequently proven to be

incorrect or false, I hereby undertake to return the refunded amount along with the applicable

interest to 'Sahara-CRCS Refund Account' and indemnify the 'Sahara CRCS Refund Account' to

the said extent. I shall also not pursue the claim in other forum to the extent it is settled by the

'Sahara-CRCS Refund Account'.

3. इसके अ तरक्त, िकसी असत्यता या िमथ्या- स्तुती क दशा म कानून के अनुसार िकसी सिवल या आपरा धक कृत्य हेतु म उ रदायी

भी रहूगं ा ।

3. Further, I shall also be liable for any civil or criminal action in accordance with law, in case of
any falsehood or misrepresentation.
