1.0 INTRODUCTION

1.1 Purpose
The purpose of this procedure is to establish the minimum requirements for the use of
Barricades, Signs and Signals on the Chiyoda, McDermott and Saipem Joint Venture
(CONTRACTOR) project.

1.2 Scope
This procedure applies to all CONTRACTOR employees, contractors, subcontractors, and
visitors utilizing barricades, signs, or signals on the CONTRACTOR site.

2.0 ABBREVIATIONS

3.0 RESPONSIBILITIES

4.0 PROCEDURE

4.1 General
Barricades shall be used in order to alert personnel to potential hazards and indicate if
authorization is required to enter the area, protective measures to be put in place and/or
protective equipment that is required.
A Barricade Matrix is shown in Appendix 1 that shall be used when determining the
appropriate type of barricade to be used. In practice, tags will be in English and Portuguese.
A barricade tag shall be affixed to each side of the barricade. The tag shall be in a
conspicuous location and shall contain the following information:
• The date the barricade was erected.
• The reason for the barricade.
• The name of the Supervisor responsible for the barricaded area and how they can be
reached (radio channel, phone number).
An example of a barricade tag is shown in Appendix 2.
Barricades shall be a minimum of 42 inches (106cm) high, neat, uniform, and level.
Barricades shall be a minimum of 3 feet (0.9m) back from the edge of excavations, holes,
or openings in platforms and roofs unless a guardrail is used. The specific distance will be
identified in WMS relevant to the task and potential hazard exposure.
Barricades shall have flashing lights when used on roadways or walkways after dark.
Barricades shall have an entrance, an opening and/or a gate whenever possible. Tape
should not be used in high wind locations or when the barricade is to be in place for longer
than 1 (one) day of work. If a barricade is to be in place longer than 1 (one) day, a more rigid
barricading system is to be used (e.g., plastic chain, heras fencing, etc.).
Barricades shall not be used for areas with a fall potential greater than 6 ft. (1.8 m). Refer
to CONTRACTOR Fall Prevention procedures for further guidance.

Barricades shall be required around certain work areas/activities as identified in the WMS.
These include, but is not limited to, the following:
• Excavations;
• Around the area under overhead work areas;
• Under elevated work areas as dropped object protection;
• Hazardous material spill locations;
• An accident scene when the site must be preserved;
• Hydro, pneumatic, and hydro-pneumatic testing of vessels and piping;
• Lifting and rigging activities to ensure personnel do not travel under suspended loads;
• Swing radius of cranes and equipment counterweights;
• Overhead power lines. Barricades, or tape shall be Red and signs shall have as a
minimum, DANGER OVER HEAD POWER LINES;
• Working near or around pressurized operating systems such as Fuel Gas, Instrument

Air, Process Water, and Steam; if the work has potential to affect personnel or industrial
operations;
• Radiography operations shall be performed by certified radiographers with appropriate
barricades, signs and survey meters.

4.2 Erecting barricades


Personnel that erect barricades shall be protected from recognized hazards while installing
and removing barricades (e.g., falling from heights, excavations, confined space, hazardous
gas vapors, vehicle traffic, heavy equipment operations, maritime, work over/around water).
Prior to erecting a barricade, the tag shall be filled out. The proper material, color, type, gates,
and posts shall be established and communicated to the personnel installing the barricade
by their supervisor.

4.3 Signs
Signs shall meet with national and internationally recognized standards. Where practical,
signs shall be pictorial and include text where required.
Signs shall be placed on barricade stands, posts, or other suitable surfaces.
Signs shall be placed in visible locations, before work starts, and must be removed to proper
storage areas when they are no longer needed.
Signs shall be legible and maintained in a usable condition.
Signs shall be pictorial and include text where required (text in English and Portuguese).
Employees shall obey posted signs.

4.4 Signals
Signals may be used in conjunction with barricades and signs to support certain activities or
routing of travelling personnel.
When signals are used to guide vehicle traffic, they shall as a minimum conform to Part VI
of the Manual on Uniform Traffic Control Devices (MUTCD).
Signaling directions and garments worn by flaggers shall as a minimum conform to Part VI
on Manual Uniform Traffic Control Devices (MUTCD).
Crane and Hoist signals to be used are found in Appendix 3.

4.5 Use of Yellow-Black Barricade Tape


Yellow-black tape is used to indicate caution or warning.
An employee may enter such an area only after reviewing the Barricade tag and taking the
necessary precautions as indicated on the Barricade tag.

4.6 Use of Yellow-Magenta Barricade Tape


Yellow-magenta tape is used to restrict access to a work area where a radioactive material
is present or where radiography is being performed.
A radiography tag shall be posted providing all necessary information.
Entry is prohibited without the authorization of the supervisor of the radiation work.

4.7 Use of Red Barricade Tape


The General Superintendent shall approve all use of red barricades on site.
Red barricade tape is used to indicate danger.
No one may enter a red barricaded area unless the supervisor signing the barricade tag
specifically authorizes entry, they have reviewed the Job Hazard Environmental Analysis
(JHEA), and the entrant has met the requirements indicated on the tag.
Tape should not be used in high wind locations or when the barricade is to be in place for
longer than 1 (one) day. If barricade is to be in place longer than 1 (one) day, a more rigid
barricading system is to be used (i.e. plastic chain, fencing panels, etc.).
When a supervisor feels that a red barricade is necessary, the General Superintendent (or
designee) shall be contacted to assess the situation. A walk down of the area will be
conducted to review the hazard. When it is not possible to eliminate or control the hazard,
the General Superintendent (or designee) shall authorize the use of the red barricade and
establish the requirements for entering the barricaded area.
A Request for Red Barricade Tape Form (example at Appendix 4), must be completed once
the General Superintendent (or designee) has deemed an area to be barricaded. The form
must be completely filled out and signed by the supervisor. Once completed with all
necessary signatures, the form will be turned into the tool room and the tool room will issue
the red barricade tape

4.8 Use of White Flagging


White flagging is used to restrict access to a work area where a pneumatic test or hydro-
pneumatic test is being performed.
Entry is prohibited without the authorization of the supervisor of the test.
Flagging will have signage indicating “testing in progress” at 25 foot intervals.
Flagging will be controlled by testing department and be erected just before commencing
test and removed at completion of test.

4.9 Use of Green Flagging or Chain


Green flagging or chain is used to designate walkways.
All Project personnel are to use the designated pathways with green flagging or chain.

4.10 Use of Blue Tape


Use of blue tape is restricted to areas handed over to commissioning to identify the area is
under commissioning control and only authorized personnel can enter the area.

4.11 Removal of Temporary Barricades


Temporary barricades shall be removed promptly by the supervisor who installed them once
they are no longer needed.

4.12 Use of Out-of-Service or Do-Not-Operate Tags


Out-of-Service and Do-Not-Operate tags are used to convey information about mechanized
equipment that are out of service or inoperable. The information that it typically conveyed
explains why equipment cannot or should not be operated, for example, a piece of equipment
may be not functioning properly, may be leaking, or may not be safe to operate.
Tags placed on mechanized equipment must be placed on a highly visible location, such as
on steering wheels, equipment controls, or ignition keys.
Tags must be filled out with the following information:
• Name and contact information of the person placing the tag;
• Date the tag was placed;
• Description of why the equipment cannot be operated or the potential hazards that exist.
Persons who must relocate, use, or repair tagged equipment shall read the tag and request
assistance from appropriate disciplines (e.g. mechanics) in rectifying the issue. If further
clarification is required, they shall contact the person listed on the tag.
Tags shall not be removed without the consent of the person listed on the tag, except as
described below.
Equipment that has been tagged with an Out-of-Service or Do-Not-Operate tag shall not be
moved, used, or operated without rectifying the issue described on the tag. This does not
apply to persons who will address/repair the issue (e.g. repair personnel or mechanics)
provided they have adequately assessed the potential safety and environmental hazards
and completed a JHEA and 5x5 for repair activities. Tags may be removed by repair
personnel or mechanics after repairs have been completed and the equipment is fit for use.
During the use of out of service tags or do not operate tags, supervisors are to ensure if any
Lock Out Tag Out is required that the requirements of CONTRACTOR Control of Hazardous
Energy Procedure MZ-000-CCX-HS-PRO-00011 are met.

5.0 POPULATION CONTROL


When lifting material overhead in a congested area, in lieu of a barricade a banksman,
wearing an orange vest using a horn or whistle, may be used to ensure no one walks under
a load. The banksman will ensure the load pathway is clear at ground level. Personnel
working at elevation along the load path shall also be cleared.
The banksman will not perform any other duties during the lift.

6.0 TRAINING
Personnel will be trained in the requirements of this procedure during site orientation training.
For specialist activities, training will be provided to personnel involved in the specific activity
(i.e. signals training for personnel conducting signalling duties, etc.).
