1. Dully filled application form of Environmental Auditor.
2. Only firm/Laboratory established in Gujarat state can apply for Environmental Auditor. 3. Rs. 20,000/‐ non refundable application scrutiny fees be sent along with this application by Demand Draft (DD) drawn in favor of ‘Gujarat Pollution Control Board’ and payable At ‘Gandhinagar’. 4. A notarized undertaking declaring “All the four members of the team are employees/ partner(s)/ Director(s) and not associated elsewhere” to be submitted on duly authenticated stamp paper of Rs.300/‐ with the photograph of the team members. 5. List of Directors/Partners…with address and Contact number. Manpower Change: 6. In case of change in manpower, auditor shall inform to the GPCB and apply within a week with all supporting document and necessary fee. 7. In case of manpower change, new team member must have minimum one year experience in the same field with requisite knowledge. Annexure I (Qualification Documents) 8. Degree Certificates of all the team members. 9. In case of B.E./B.Tech in Civil Engineer a document showing the specialization in environmental engineer as an elective subject. Annexure II (Appointments Documents) 10. Appointment letter of all team members from organization. Annexure III (Experience Documents) 11. Experience related documents of all team members. Annexure IV (Infrastructure Details) 12. Laboratory Sketch ‐ for fresh application/ change in laboratory location only. 13. List of Laboratory instruments available at a time of application. 14. List of Parameters for which laboratory is capable to analyze for Water, Wastewater, Hazardous Waste, Ambient air, Stack, Noise etc. Annexure V (Other Details) 15. Name of the In‐charge of the firm along with the Mobile Number and E‐mail Id. 16. Past Recognition of Environmental Auditor – for Renewal and Amendment application. 17. Following detail shall be submitted in case of renewal for current and previous year. i. Numbers of Industries Allotted. ii. Numbers of Industries (Audit carried out). iii. Numbers of Industries (Audit not Carried out). iv. Name of the industries with reason for not carrying out audit. 18. Past works in the field of Environment Management System‐ for Fresh application. 19. List of Clients for whom work w.r.t environment field is done – for Fresh application. 20. Any other remarkable work in the field of environment management system. 21. Any incomplete application shall be rejected. 22. The applications for fresh recognition as Schedule I or II environment auditors shall be accepted by Gujarat Pollution Control Board throughout the year but the fresh applications received after 31st of December shall not be considered for allotment for the subsequent financial year.
