DepEd School Form (SF) 2 Review

Every teacher is required to submit their reports throughout the school

year. These reports include most of the data related to the schools and
students such as class records, IPCRF and other documents. Therefore, the
Department of Education (DepEd) officially released these school forms to help
teachers consolidate the data for the whole school year. School forms are necessary
to both the school and the learners as it is where their records can be seen.
Rationale All the educational background significant to the students’ education are
compiled for documentation and future purposes. The preparation and
checking of school forms, undertaken to ensure the quality and
consistency of learner information, are among the critical activities conducted at
the end of every School Year (SY).
Accomplishment of school form 2 is one important tool in monitoring the status of
learner to immediately provide intervention or action for those learners who will be
identified as at risk of dropping out.
 For consistency on the encoding of School Form 4, grade level ICT
coordinators will be reoriented on the proper way to enter needed data
Learning Goals on the said form.
 Participants will have a workshop on encoding and making entry on
(Skills/Competencies)
DepEd school form (SF) 2.
 At the end of the session, participants will be equipped with the
knowledge on the proper way of using school form (SF) 2.
