JOINT AFFIDAVIT
(RE: CHANGE OF VENUE OF MARRIAGE SOLEMNIZATION)

We, JEROME C. SOLPICO and KATHLEEN T. DIZON, Filipinos, both of legal

age, single, and respectively residing at 10764 M. Escobin St. Mayondon Los Baños, Laguna
and 9435 Garcia Cmpd., Lopez Ave., Batong Malake, Los Baños, Laguna, after having been
duly sworn to in accordance with law, do hereby depose and state that:

1. We are getting married on September 27, 2023;

2. We have requested REV. ALEXANDER M. OCAMPO, Senior Pastor of Zion Hill

Bible Baptist Church, Cuenca, Batangas, Philippines as the solemnizing/officiating
minister of our marriage;

3. We intend to have our wedding be solemnized by Rev. Alexander M. Ocampo at

ONE B PARTY PLACE & EVENTS, MANAOAG, PANGASINAN on the aforecited date
and time;

4. We are executing this affidavit to attest to the truth and veracity of the foregoing
statements and for the purpose of compliance with Article 8 of the Family Code of the
Philippines which states that marriages “shall be solemnized publicly in the chambers of the
judge or in open court, in the church, chapel or temple, or in the office the consul-general,
consul or vice-consul, as the case may be, and not elsewhere, except in cases of marriages
contracted on the point of death or in remote places in accordance with Article 29 of this Code,
or where both of the parties request the solemnizing officer in writing in which case the
marriage may be solemnized at a house or place designated by them in a sworn statement to
that effect; and for whatever legal purpose it may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this 19th day of September
