The Admissions Committee of La Salle College Antipolo would like to inform you that your child, Ariana Necole A.
Nemenso is accepted with condition for Grade 11, STEM Strand, for School Year 2023-2024.

Based on the admissions result, your child will be placed under Academic Probation for the 1 st quarter of the
school year. The 1st quarter grade in Math and English subject will determine if the status will be lifted or if your
child will undergo Remediation Program during the 2nd quarter.

In pursuing the enrollment of your child at La Salle College Antipolo, please be guided by the following procedure:

1. Sign the Academic Probation Contract.

2. Enrollment is ongoing. Please ensure that the following have been submitted to the Admission Office before
proceeding to the Registrar’s Office for enrollment.
 Form 138 or Original Report Card. Only Report Cards that are duly signed will be honored for
enrolment.
Other required documents (if applicable)
 PSA Birth Certificate (photocopy)  Certificate of ESC Grant/QVA Certificate
 Baptismal Certificate; or its equivalent (photocopy)  Two Recommendation Letters
 Certificate of Good Moral Character (for enrollment/  Medical Certificate and Chest X-Ray
transfer purposes) result

3. Please see attached Assessment of Tuition and Fees.

4. Assessment for the required materials and uniforms will be available by June 2023 at the LSCA

5. Attendance at the Orientation for New Students and their Parents is a must. You will be notified of the
schedule by our office.
