 Approved Form 6 of the Teacher on Leave (1 copy)
 Photocopy of PSA Birth Certificate, for married women, Photocopy of PSA Marriage Certificate (1 copy)
 Notarized Personal Data Sheet (PDS) or CSC Form 212 Revised 2017 Properly Accomplished with Identical
Passport Size Picture taken at authorized Photo Studio. Scanned or Computer generated picture IS NOT
ACCEPTABLE (can be downloaded at https://www.facebook.com/SDOTarlacPersonnel) (4 original copies)
 Work Experience Sheet (can be downloaded at https://www.facebook.com/SDOTarlacPersonnel) (4 original
copies)
 Photocopy of Official Transcript of Records (Doctorate, MA & Bachelor’s Degree) certified by the University
Registrar (1 original copy and 2 photocopy)
 Updated PRC License-photocopy duly authenticated by the Professional Regulation Commission (1 original copy
and 2 photocopy)
 Board Rating (3 copies)
 NBI Clearance (1 original copy and 2 photocopies)
 CSC Form 211 (can be downloaded from https://www.facebook.com/SDOTarlacPersonnel) (Philippine Civil
Service Medical Certificate) duly signed by a Government Physician ( 2 original copies)
 Complete medical results (Neuro-Psychiatric Test, Blood test, Urinalysis, Drug test and Chest X-ray) (Can be
downloaded at CSC website www.csc.gov.ph) (1 copy)

Non-compliance with the submission of the aforesaid requirements shall exonerate this Office from
administrative responsibilities and accountability.
