I. Title : 45th National Disability Prevention and Rehabilitation Week


Celebration
II. Theme : Persons with Disabilities Accessibility and Tights: Towards a
Sustainable Future Where No One is Left Behind

IV. Amount Requested : ₱ 143,770.00
V. Source of Fund : PWD Program
VI. Rationale :

By virtue of Proclamation no. 361, the State declares the 3 rd week of July as the
National Disability Prevention and Rehabilitation Week. It is the national interest to stimulate
public awareness on the problems of disability, and encourage every citizen to take active
responsibility in the upliftment of the economic and social conditions of the PWDs of our
society.

By understanding the needs and capabilities of PWDs, opportunities will be created


by citizens and the government, thus, making PWDs more productive, self-reliant, and
empowered to participate as active citizens contributing to the betterment of society and the
development of the economy.

With this, the Municipal Social Welfare and Development Office joins the National
Disability Prevention and Rehabilitation Week Celebration. However, the said celebration will
be conducted on July 26, 2023 instead of the 3 rd week.The activity will commence Health
and Wellness Program. During the event, there will be PWD Got Talent, board games
(chess and dama), poster making contest and medical services (medical consultation, dental
services, blood sugar testing, blood typing and eye check-up.

VII. Objectives:

1. To serve as the vehicle of information for programs implemented by our


government for persons with disabilities
2. To encourage every citizen to take active responsibility in the upliftment of
persons with disabilities thereby encouraging them to take pro-active roles in
nation-building.
3. To stimulate public awareness on disability problems and concerns
particularly issues on disability prevention, rehabilitation and equalization of
opportunities for persons with disabilities.
VIII. Budgetary Requirements:

 Lunch : 350 pax (P175.00 per pax) : 61,250.00


 Snacks (AM & PM) : 50 pax (P150.00 per pax) : 7, 500.00
 Stage Decoration : 5,000.00
 Tarpaulin (size 4x8) :1pc : 480.00

IX. Recommendation:

In view of the above premise, the undersigned persons herby recommend that
the amount of ₱143,770.00 pesos only be approved to defray the aforementioned
expenses.
