    EVALUATION OF DOCUMENTS
    a. Evaluate grades based on presented school credentials
    - grades from Transcript of Records (TOR)
    - subject's for substitution
    - subject/s for enrollment at ETEEAP

    3. ENROLLRENT AT THE UNIVERSITY REGISTRAR


    Applicants are required to submit the following documents:
    - Birth certificate PSA authenticated
    -High school card/Form l37/Transcript of Records/Honorable Disrissal
    - 2 pcs. 2x2IDpicture
    - Student Directory
    - Entrance Health examination slip

    5. PAYRENT AT CASHIER'S OFFICE

    6. APPLICATION FOR GRADUATION


    - Applicant should submit accomplished Application form for
    Graduation at the ETEEAP office

    7. RECOMREND FOR GRADUATI`ON


    - The University Registrar recommends the appl].canals for graduation to the Academic
    Council and the letter to the Board of Regents.

    Tuition fee 500.00/unit
    Program Adviser - 3, 000.00
    Miscellaneous fees 600.00
    Tutorial services - 250.00/hour
