INFORMED CONSENT FORM

You will be asked to participate in an interview regarding your study habits and academic
experiences. Your participation will involve a single interview session, estimated to take
approximately 2 hours and 30 minutes. Your identity and personal information will be kept
confidential. Your responses will only be accessible to the researchers and will be used for
research purposes only. Under the Republic Act No. 10173, Chapter II, Section 8 and Chapter
III, Section 12. A and Section 13. A, your data will be handled with utmost confidentiality, and
failure to comply with the law could result in penalties.

With your consent, the interview will be audio-recorded and/or video-recorded for
accurate transcription and analysis purposes. The recording will be securely stored and only
accessible to the researchers and the research adviser for checking the research paper. The data
collected from the interviews will be analyzed using a descriptive phenomenological analysis
approach to understand the themes and patterns related to study habits and academic
achievements.

Participating in this research study will:

1. Have a reflection on your college days, especially your academic journey and study
habits. Potentially leading to insights and how you progressively grow in an academic
setting.
2. You will be a key part of the research study titled “Understanding Effective Study
Habits in the Academic Achievements of National College of Business and Arts
Cubao Branch’s Higher Education Graduates with Latin Honors”, through your data
and your experience with your study habits.

Your participation in this study is entirely voluntary, and you have the right to withdraw
at any time without consequence. Your decision to participate or withdraw will not affect your
current or future relationship with the researchers or the institution.
