DISCHARGE PLAN

Health Teaching

● Encouraged the relatives to make the patient's environment safer by placing needed items

within arms' reach.

● Educated the patient and relatives about the importance of complying with taking medications

on the right time and frequency, having a proper and balanced diet, and the benefits of exercise.

● Encouraged the patient to have adequate sleep to have enough rest.

● Advised the patient to lessen emotional stress as it precipitates abdominal pain

● and stress.

● Advised the patient to wear comfortable, well-fitting shoes to minimize the risk of injuries to the

feet.

● Advised the patient to monitor the blood sugar, visit the barangay health center to determine

the status of blood sugar.

● Demonstrated and guided the patient in safe practices during personal hygiene activities,

emphasizing gentle handling and avoiding sharp tools.

Anticipatory Guidelines

● Watch out for Abdominal Pain

● Watch out for Fever (37. 8℃) or higher

● Watch out for pitting edema

● Watch out for difficulty of breathing (DOB)

Spiritual
● Provided genuine emotional and spiritual support.

● Urged the family members to engage in prayers for the patient's previous recovery and overall

well-being.

● Advised the patient's relatives to be present in order to prevent a sense of helplessness.

Medications

● Provided the patient with instructions on how to take their prescriptions.

● Instructed the patient to keep an accurate medicine list and current dosage at all times.

● Instructed the patient not to self-medicate mainly when taking antibiotics to prevent antibiotic

resistance. This precaution helps preserve the effectiveness of antibiotics in treating bacterial

infections, ensuring they remain a viable and potent form of medical intervention.\

● Instructed the patient and the relative to incorporate the therapeutic plan into their everyday

activities to achieve the desired therapeutic effect.

Incision Care

● Shower regularly with warm water then pat gently on the abdomen and feet using antimicrobial

soap.

● Apply moisturizing lotion in skin areas to prevent dry skin.

● Use sterile technique to minimize the risk of infection

Nutrition

● Recommended that the individual consume foods with reduced fat and cholesterol content,

such as fish, chicken breast, and tofu.

● Advised the patient to steer clear of foods high in salt and fat and to refrain from dining at fast-

food establishments.
● Instructed the patient for periodic follow-up appointments to evaluate the patient's general

health status, attend to any issues, and strengthen safety measures.

● Encouraged to continue drinking an adequate amount of water.

● Advised the patient to abstain from alcohol.

● Advised the patient to use the DASH DIET (Dietary Approaches to Stop Hypertension)

- To promote a balanced diet rich in fruits, vegetables, whole grains, and low- fat dairy.

● Encouraged the patient to eat foods rich in iron, such as dark- leafy vegetables and fruits.

Environmental/ Exercise

● Advised the patient not to engage in activities that require strength.

● Encouraged deep breathing exercises (DBE) to enhance lung function and oxygenation.

● Initiated a supervised walking program, gradually increased the duration and distance as

tolerated. This helps improve overall cardiovascular fitness.
