Kwestionariusz Zaburzeń Nastroju - Charakterystyka I Zastosowanie
Summary
Bipolar disorder (BD) is one of the most difficult to diagnose among all psychiatric dis-
orders. BD is often misdiagnosed as recurrent depressive disorder, schizophrenia, anxiety
or personality disorders. In 40% of patients BD is not detected at the first examination and
in 2/3 of cases the proper diagnosis is made after about 10 years. The consequence of this
condition is inadequate treatment leading to the worsening of the course of BD or increased
risk of substance misuse and suicidal behavior. Those data reveal that the better detection of
bipolar spectrum is strongly needed. It may be achieved by the application of the Mood Dis-
order Questionnaire (MDQ) in clinical practice. MDQ is a self rating questionnaire created by
Hirschfeld et al. for screening of bipolar spectrum. According to previous data the usefulness
of MDQ in populations of psychiatric patients (especially those with affective disorders) is
unquestionable high. The significant limitation of MDQ is a relatively low sensitivity for BD
type II. That’s why the concomitant use of MDQ (as an excellent tool for detection of BD
type I) and the other instrument characterized by the higher sensitivity for hypomania and
BD type II (e.g. The Hypomania Symptom Checklist by Angst et al.) seems to be the best
and recommended solution. This article describes the properties and structure of MDQ. The
capabilities, advantages and limitations of MDQ were also presented. The authors discussed
the results of validation studies of different language versions of MDQ and summarized –
DEP-BI and TRES-DEP – Polish studies using MDQ.

zaburzeń dwubiegunowych typu II. Wielu pacjentów dotkniętych ChAD-II błędnie uważa
występujące u nich fazy hipomaniakalne za okresy normalnej i zdrowej aktywności, co jest
czynnikiem istotnie utrudniającym postawienie właściwej diagnozy i przyczyniającym się
do jej znaczącego opóźnienia. Niestety, KZN w tych wypadkach może się przyczynić do
postawienia diagnozy fałszywie ujemnej, zwłaszcza w razie niewystarczającej czujności
diagnostycznej lekarza. Nie dysponujemy obecnie narzędziem diagnostycznym, które łą-
czyłoby w sobie prostotę, łatwość i małą czasochłonność z wszechstronnością pozwalającą
na pewne wykrycie wszystkich odmian spektrum zaburzeń dwubiegunowych. W tej sytu-
acji wydaje się zasadne łączenie KZN (jako kwestionariusza przydatnego w wykrywaniu
ChAD-I) z innymi narzędziami, które wykazują większą czułość w stosunku do hipomanii
czy ChAD-II, takimi jak, stworzona przez Angsta i wsp., Hypomania Checklist (HCL-32)
czy Bipolar Spectrum Diagnostic Scale (BSDS) opracowana przez Ronalda Piesa [za:
29, 53, 54]. Należy również dodać, że obecnie opracowywane są i weryfikowane nowe
kwestionariusze, mające służyć diagnostyce przesiewowej zaburzeń dwubiegunowych
oraz różnicowej zaburzeń jedno- i dwubiegunowych. Za przykład może tu posłużyć Mood
Swings Survey (MSS). Użyteczność kliniczna i konkurencyjność tego narzędzia w stosunku
do KZN wymaga dalszych badań [55].

Двухполюсная аффективная болезнь – ДАфБ составляет большие диагностические
трудности. Эта болезнь часто ошибочно диагностируется как рецидивирующая депрессия
или фобийные нарушения, шизофрения и нарушения личности. У 40% пациентов болезнь не
диагностируемая во время первого обследования, а установление правильного диагноза в 2/3
случаев наступает после почти 10 лет. Результатом этого явления является неправильное лечение
и, как следствие, утяжеление течения болезни, рост риска фармакомании и суицидального
поведения. Все это указывает на необходимость более внимательного подхода к распознавание
спектра ДАфБ. Одним из способов достижения такого задания может быть применение
Глоссария нарушений настроения (ГНН), который является глоссарием самооценки,
служащим для скриннингового определения спектра ДафБ. Из известных исследований
истекает, что пригодность ГНН в исследованиях пациентов страдающих психическими
нарушениями (особенно аффективные), очень высокая. Однако, существенной проблемой
является относительная чувствительность ГНН для диагностики ДАфБ второго типа. В этой
ситуации, по-видимому, необходимо применение глоссария ГНН (как особенно эффективного
для диагностирования ДАфБ I типа) с иными пособиями с большей чувствительностью
по отношению к гипомании или ДАфБ II типа (например приготовленная Ангстом и сотр.
Hypomania Checklist).
Представляемая статья описывает характеристику и структуру ГНН. Представлены
возможности использования пособия, обсуждены его пороки и пригодность. Авторы
представили также результаты подобных исследований в разных языках ГНН, а также
обсудили польские исследования в различных центрах – DЕР-ВI, а тоже ТRЕS DEP – в которых
использовался ГНН.

und richtige Diagnosestellung erfolgt bei 2/3 der Fälle nach ca. 10 Jahren. Die Folge dieser Erscheinung
ist eine nicht richtige Behandlung, die nach sich die Verschlechterung des Krankheitsverlaufes,
erhöhtes Risiko für den Mißbrauch von Substanzen und Suizidversuchen zieht. Es zeigt, dass es
notwendig ist die Entdeckung des Spektrums der affektiven zweipoligen Krankheit zu erhöhen.
Eine der Methoden zum Erreichen dieses Zwecks kann die Anwendung in der klinischen Praxis des
Fragebogens von Hirschfels und andere “Fragebogen zur Erfassung von Stimmungsschwankungen”
sein. Der Fragebogen ist ein Fragebogen zur Selbstbeurteilung zur Entdeckung des Spektrums der
affektiven zweipoligen Krankheit. Aus den bisherigen Studien geht hervor, dass die Brauchbarkeit
des Fragebogens bei der Untersuchung der Patientenpopulationen, die an psychische Störungen
leiden (besonders affektive) ist ohne Diskussion sehr hoch. Ein wesentliches Problem ist aber eine
verhältnismäßige niedrige Empfindlichkeit auf zweipolige affektive Krankheit vom Typ II. In dieser
Situation scheint es begründet zu sein, die Verbindung dieses Fragebogens mit anderen Werkzeugen,
die eine größere Empfindlichkeit auf Hippomanie oder zweipolige affektive Krankheit vom Typ II
zeigen (zB. Hypomania Checklist von Angst). Der vorliegende Artikel beschreibt die Charakteristik
und Struktur des Fragebogens zur Erfassung der Stimmungsschwankungen. Die Möglichkeiten seiner
Anwendung wurden dargestellt, seine Vor– und Nachteile besprochen. Die Autoren beschrieben
außerdem die Ergebnisse der validativen Studie unterschiedlicher Sprachversionen des Fragebogens
und besprachen polnische mehrzentrale Studie – DEP–BI und TRES–DEP, bei denen der Fragebogen
angewandt wurde.
