AFFIDAVIT
I, EMELY DALAGAN PRADEZ, of legal age, Filipino,
married, and a resident of Barangay Cay Pombo, Sta. Maria,
Bulacan, under oath, state that:

1. I am one of the children of the late FELIPE CURBA

DALAGAN, who was born on January 1, 1921 at
Tagbilaran, Bohol;

2. My father, FELIPE CURBA DALAGAN, died on June 4,

1982 in Baler, Aurora;

3. The cause of death of my father was due to heart failure;

4. The fact of death of my father was not registered to any

proper government agency;

5. I am executing this affidavit to attest to the truthfulness

of the fact of the death of my father, Felipe Curba
Dalagan, and for whatever legal purposes that this
statement may serve.

IN WITNESS WHEREOF, I have hereunto affixed my
