address at 307 Sandico St., Poblacion II , Marilao Bulacan, have named, constituted and appointed,
and by these present do hereby name and constitute, and appoint _Leonel A. Hipolito_, likewise of
legal age, single/married, with residence and postal address at #83 P. Faustino st. Punturin Valenzuela
City 1447, to be my true and lawful Attorney-In-Fact, for me and in my name, place, and stead, to do and
perform any and all of the foregoing acts and things, namely:

1. To deal/negotiate all transactions with the Bureau of Internal Revenue with my tax compliance.
2. To perform my business closure in BIR.
3. To be authorized to apply, sign, execute, enroll, follow-up and deliver any and all documents
regarding the registration, closure and monthly compliance in BIR.
4. To make, sign, and execute for in my behalf, any and all documents evidencing receipt of my
aforesaid request.
