(Absence accident )
Case : The victim's was repairing it using a gas welding equipment. The supply hose came off, flame ignited blown-out the right arm.
Location : Parts Supply Gate Date & Time : Apr 24th ,2022 at about 8:20 pm Injury Level : Right arm 1st degree burn

Detail of accident Factors

The victim was repairing the upper rail of the parts supply gate was bent
and the gate did not close.
(1) The victim was repairing it using a gas welding equipment.
(2) When the acetylene supply hose came off the blow tube, the flame
1) Pre-operation check of the equipment was omitted .
2) The victim had noticed a failure of the acetylene pressure gauge,
but continued use it.
3) The victim did not wear the specified protective equipment
(gloves, arm covers, apron) but wore short sleeves.

Countermeasure
1) Pre-operation check was thoroughly conducted.
2) It was ensured to report an equipment failure to a superior and not
to use the broken equipment.
3) Prepare a protective equipment checklist and check it before work

Essence : The victim doesn't check equipment before work and not
wear the specified protective equipment.
