Subject: Design and Construction of Viaduct in Reach-3 between Jhansi Rani Square and
Lokmanya Nagar Station from Ch.7825m to Ch.18212m on the East-West Corridor
of Nagpur Metro Rail Project- Safety Show Cause Notice.

Dear Sir,

Inspite of repeated observations / instructions (Verbal/ Written) given to you on various

occasions for strict adherence/ compliance to clauses of SHE Manual of Contract Agreement,
no improvement/ efforts for compliance is seen for day to day construction & its associated
works.

We re-iterate that these deviations to Part - III, Section - XI: Safety Health & Environment of
NMRCL COC is a serious misconduct. Despite the fact that our Engineers have been
highlighting the same time & again, but no improvement effort is being observed from your
end.

Further in light of the incident today wherein your Hydra Crane Reg No: MH 49 AK 0018 has
been found transporting the workers in hazardous manner, No Third Party Test certificate &
ASLI Calibration certificate available for Crane and Crane operator not wearing the
Mandatory PPE during working hours. You may please treat this letter as show cause notice
for not imposing penalty as per the Clause 56.3: Charges to be recovered from the
contractor for unsafe act or condition, Sub Clause under topic Sr. No 14 (v, vi, xi) and 25 (i) on
failure to adhere clause 23.4 (ii), 21.7, 21.8 & 39.0 of Part - III, Section - XI: Safety Health &
Environment of NMRCL COC.

You are instructed to submit the detailed Action report within 3 days from receipt of this
show cause notice, failing which GC will proceed further.

Safety Action:

 No Passenger shall be carried, unless specific

seating has been provided in accordance with the
Manufacture recommendations.
 Every lifting appliance shall be tested and
examined by the competent person at least every
6 months.
 Every lifting appliance shall be fitted with ASLI,
calibrated by IS/ International certifying bodies.
 The contractor shall provide required PPE to
workmen to protect against safety and/ or health
hazard.
