Spirit-filled greetings!

I am Keycie G. Perez, a Teacher 1 of Don Amancio Bendigo Sr. Integrated School and have been
teaching Grade six for five years and five months.

This is to express my intent to seek transfer from the present school Don Amancio Bendigo Sr.
Elementary School to Inawayan Elementary School for the following reason as stated herein
based on DepEd Order No.22, s. 2013:

 I have a newborn son and I’m a nursing mom, as a mother it is my desire to take
good care of my child. Furthermore, Don Amancio Bendigo Sr. IS is very far away
from my residence and it would be impossible for me to be at our home early.
Hence, It would be a great help if I will be assigned near to my residence; and

 I am a currently residing at Purok 9, Baracatan Toril Davao City which nearby the
school that I want to be transferred.

I am fervently hoping and praying that this request be granted and be given attention. If
granted I will be forever grateful for your consideration and understanding my situation.
