OSCE Checklist: Mental Capacity Assessment

Opening the consultation


1 Wash your hands and don PPE if appropriate

2 Introduce yourself to the patient including your name and role

3 Confirm the patient's name and date of birth

4 Explain the reason for the consultation

Conducting the capacity assessment


5 Assess if the patient can understand the situation and the options presented to them

6 Assess if the patient can retain this information

7 Assess if the patient can use this information to make a decision

8 Assess if the patient can communicate their decision

Closing the consultation


9 Thank the patient for their time

10 Dispose of PPE appropriately and wash your hands

11 Document the mental capacity assessment in the medical notes
