Congratulations! We are pleased to inform you that you will be part of Santé Regional
Travel – Asian Cruise! The trip is set for March 17-20, 2024.

With this, we request to submit the following requirements:

a. Intent-To-Join REPLY SLIP

Please send us the filled-out reply slip (found on the 2nd page of this letter).
Kindly submit the reply slip no later than January 26, 2024.

b. Scanned copy of Passport

For passports expiring earlier than March 2024 we strongly recommend that you
apply for a new one since it is required that a passenger must have a valid pass
port not expiring less than six (6) months upon travel.

Please make sure that you send clear scanned copies. Non-submission of the Reply
Slip and Copy of Passport to Santé on or before January 26, 2024, or non-compliance
will automatically forfeit this trip.

I, Phoebe T. Daugdaug, an active Business Owner of Santé, hereby confirm my

intention to join the scheduled trip for Santé Regional Travel – Asian Cruise between
March 17-20, 2024.

I understand that non-submission of required documents and non-compliance will

automatically mean I have waived my rights to join the said trip.

Furthermore, I commit to confirming my participation in the Santé Regional Travel –

Asian Cruise. However, I acknowledge that after submission of this letter and
documents, if I request to cancel my trip, a Php 50,000 cancellation fee will be
charged through automatic commission deduction. This fee is exempt in cases of
cancellations due to health issues, supported by a medical certificate, or emergencies.
