AFFIDAVIT OF CITIZENSHIP

I, MONALIZA PIEDAD VISTA, of legal age, Filipino citizen,

married with postal and residence address at No. B 7 L 53 Villa
Celina Subd., Brgy. Bayan Luma 7, Imus City, Cavite, after having
been duly sworn to in accordance with the law, depose and say:

1. That, I was abandoned together with my children by my

former husband in 1997;

2. That, I met BENJAMIN DOMASIAN GONZALES, legal age,

Filipino-American citizen, sometime in 1998;

3. That, we have been living together as common-law-partners

under the same roof, continuously and without any
interruption, since January 1999;

4. That, we were blessed with one (1) child namely BENJAMIN

DELA CRUZ PIEDAD, who was born on June 13, 2000;

5. That, the said child acquired his American citizenship on

February 6, 2001, and the name BENJAMIN DELA CRUZ
PIEDAD was changed to BENJAMIN PIEDAD GONZALES,
JR.;

6. That, the names BENJAMIN DELA CRUZ PIEDAD and the

BENJAMIN PIEDAD GONZALES JR., are one and the
same persons;

7. That, I am executing this sworn affidavit to attest the

foregoing facts in support of my application for recognition
as a Filipino citizen and for other lawful purposes.
