AFFIDAVIT OF INCIDENT

I, DANILO QUILATAN, of legal age, Filipino citizen, married

and with postal and residence address at No. 0092 Burgos St.,
Brgy. Bahayang Pag-asas Ph 8-B, Magdalo, Imus City, Cavite, after
having been duly sworn in accordance with the law, depose and
state:

1. That, I am a subscriber of Meralco electric company with

Customer Account Number CAN 1245351803 and Meter
device No.: 23GM005268;

2. That, on November 16, 2023, at about 3:30 P.M., there was

a construction work at the abovementioned residential
house. A construction worker works at the second floor
when he inadvertently lost in hand a metal rod from his
location that fell on a meter device which was installed on a
concrete post nearby the house. The abovesaid meter
device’s glass was broken;

3. That,
the afore-stated electric meter device is totally
damaged;

4. That, I have reported the said incident to the Meralco

through a phone call and I was advised to secure a Police
Report and together with a notarized Affidavit of Incident;

5. That, I am executing this Affidavit to attest to the

truthfulness of the foregoing facts and for whatever legal
purpose it may serve.
